Should I call Quest before making an appointment?

Do I need a referral from my doctor to see a Quest provider?
Members do not need a referral from your doctor for behavioral health or substance abuse appointments or admissions. Quest does encourage ongoing communication between behavioral health or substance abuse providers and the member’s physicians to facilitate coordination of care.
Do I need to preauthorize my appointments?
Quest preauthorizes EAP counseling, Psychological Testing, and higher level of care services (inpatient, intensive outpatient, partial hospitalization). Regular outpatient appointments for which EAP benefits are not being used do not require any preauthorization.
How do I determine if my provider is in the Quest Provider network?

What if I want to see someone not in the Quest Behavioral Health Provider network?
Call Quest to determine your out-of-network benefits and financial responsibility. Members may see a provider that meets Quest provider credentialing standards. Reimbursement will be based on Quest fee schedules.
Note: Out-of-network providers are not required to accept Quest rates as payment in full and may balance bill members for the remaining financial obligation, which may result in a higher out-of-pocket cost. Not all plans have out-of-network benefits. Members with no out-of-network benefits will be responsible for payment in full.
