Becoming a Quest Provider

Quest’s premier provider network boasts participation by Central Pennsylvania's leading health systems, a wide range of treatment facilities, groups and more than 1,110 individual professional providers. The ever-growing network is supported by Quest’s continuously expanding client base.

If you are interested in joining the Quest provider network, please click on the electronic pre-application summary form to the right. The Quest Credentialing Committee will review and assess completed pre-application summary forms for eligibility and necessity. Pre-applicants that are eligible will receive a full credentialing application within thirty [30] days of committee decision. Interested providers may also contact the provider relations department at 717.851.1478 with additional questions or inquiries.   

Please note participation in the Quest network requires the following:

  • Minimum of a Master's degree in a behavioral health or human services field
  • Minimum of 5 years post Master's experience
  • Minimum malpractice coverage of $1 Million/$3 Million
  • Current state license/certification and DEA licensure if M.D.
